We see patients of all ages.

Seniors should have an eye exam every year especially if they have Chronic diseases including Cataracts, Diabetes, High Blood Pressure, Macular Degeneration, High Cholesterol, Hearst Disease, Glaucoma, and Dry Eye Syndrome.

WIC is a nutrition education program for Women, Infants, and Children.

You May Qualify If You:

• Are pregnant, breastfeeding, or just had a baby;
• Had a recent pregnancy loss;
• Have a child or care for a child under 5;
• Have low to medium-income; and/or
• Receive Medi-Cal, CalWORKS (TANF), or CalFresh
(SNAP) benefits; and live in California

What is Case Management?

During an appointment, the following may happen depending on the needs of the patient.
• We will provide you all the information you may need for our clinic services like Medical, Dental, Vision, and WIC.
• If applicable will be offered to you the insurance or health program may cover better your needs; refer as needed to, the in-house Medi-Cal worker
• Medication management for patients who qualify for financial assistance programs
• Referral appointment management & coordinate referral authorizations
• Counsel on diet and lifestyle management
